COMMUNITY OVERVIEW

Humber River Family Health Team (HRFHT) offers a wide variety of specialized services to the patients.

 

The program and services are delivered in a team based care model that includes physicians and an in-house clinical team of Health Care Professionals  (i.e. Nurse Practitioners, Registered Nurses, Dieticians, Social Workers, Chiropodist, Health Promoter/Educator and Pharmacists) alongside other community service providers

 

The medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioners, dietitians, diabetes educator, occupational therapist, chiropodist (foot care specialist), health promoter/educator, and social workers
